Risperidone-D4 is a long-acting injectable formulation of risperidone that was developed to address the limitations of oral administration. It is administered intramuscularly and has a duration of action of up to 2 weeks. Risperidone-D4 can be used for the treatment of schizophrenia, bipolar disorder, and major depressive episodes in adults with psychotic features. The pharmacokinetics are linear and dose proportionate over the therapeutic range, with high bioavailability and low protein binding. The elimination half-life is approximately 10 days, which allows once-weekly dosing. Risperidone-D4 is metabolized by esterases in the blood and liver to inactive metabolites, which are eliminated by renal excretion. The drug is not significantly absorbed into erythrocytes or other tissues, so it does not accumulate in these tissues over time.
