Nitrofurantoin's mode of action involves the inhibition of essential bacterial enzymes. It is reduced by bacterial flavoproteins to active intermediates, which subsequently alter or disrupt ribosomal proteins, DNA, and other vital macromolecules within bacterial cells, thereby inhibiting key metabolic processes. Nitrofurantoin is most commonly used in the treatment and prophylaxis of uncomplicated urinary tract infections (UTIs). Its effectiveness is primarily restricted to the urinary tract because it achieves therapeutic concentrations only in urine. It is particularly active against Escherichia coli and some strains of Enterococcus, Klebsiella, and Staphylococcus. Due to its targeted spectrum and localized action, it is generally reserved for infections caused by susceptible organisms within the urinary tract, reducing the risk of systemic side effects.
