Hidrato de tacrolimus, con el número CAS 109581-93-3, es un agente inmunosupresor utilizado principalmente en el trasplante de órganos y enfermedades autoinmunes. Se deriva de la bacteria Streptomyces tsukubaensis y funciona inhibiendo la activación de los linfocitos T, reduciendo así la respuesta inmune. La sustancia aparece como un polvo blanco a blanco amarillento y es soluble en disolventes orgánicos como el metanol y el dimetilsulfóxido, pero tiene una solubilidad limitada en agua. Hidrato de tacrolimus a menudo se formula como un ungüento tópico o medicamento oral, dependiendo de su aplicación. Sus efectos farmacológicos se atribuyen a su capacidad para unirse a FKBP-12, una proteína que posteriormente inhibe la calcineurina, una enzima crítica en la activación de células T.
